Prostate Cancer Survey

Prostate Cancer UK's first National Survey of Men with Prostate Cancer in Scotland.

This questionnaire is about your care and treatment for prostate cancer. Its purpose is to provide information which can help the NHS and Prostate Cancer UK monitor and improve the quality of health services for future patients.

Who should complete the questionnaire?

The questions should be answered by you as the person who has been treated for prostate cancer. If you need help to complete the questionnaire, the answers should be given from your point of view – not the point of view of the person who is helping.
